NATIONAL CONFERENCE IN PERSONALISED CARE

We are delighted to announce our second national conference in personalised care themed around working together – integrated care teams, to be held in-person in the vibrant city of Sheffield, South Yorkshire. The event is aimed at health and wellbeing coaches (HWBC), care co-ordinators, link workers (SPLWs) and their management as well as being relevant to those in wider community, policy and voluntary roles. The day will include a focus on the value of creating and maintaining integrated personalised care teams. CPD on this day will include applying lifestyle medicine approaches, working in groups, developing coaching practice and new thinking regarding chronic pain. It will be an important opportunity to connect with others in the field of person-centred care and to think collectively about how to create and sustain a coaching culture in the NHS.

​The conference aims are to:

​

• increase shared knowledge and understanding of the value of a coaching mindset for individuals and society 

• focus on chronic pain and how we can work differently to support patients 

• support delegates in gaining new knowledge, skills and tools for health coaching practice with individuals and in groups 

• raise awareness of the benefits of lifestyle medicine approaches 

• contribute to building and sustaining a team approach to person-centred care.
